Instructor Web Page Account Request

Web accounts are available to Staff and Faculty of West Valley College.

In order to set up an account on the INSTRUCT.WESTVALLEY.EDU web server, you must:

• Read and understand the WVC HTML CONVENTIONS page.
• Read, understand, and agree to comply with the WVMCCD INTERNET USE POLICY.
• Print and sign a copy of the ACKNOWLEDGMENT signifying your agreement.

Once you have printed and signed this form please bring it to David B Duncan, College Webmaster in his office,
Fox Center 206 map, or send by inter-office mail.

Your account, FTP rights and default password will be established within 5-7 days after you turn this signed form in.

You will be notified by e-mail when your account has been established and is ready to use.
