Care Report
Report a concern about a student’s well-being. This form can be used to connect students to resources like health and wellness, services, financial services, academic resources, DESP, and EOPS.
Incident Report
Report a safety or behavioral concern including unlawful harassment/discrimination/retaliation, disruptive behavior, academic dishonesty, student grievances, or COVID-19 concerns.
Title IX Report
Report sex-based harassment or discrimination including sexual harassment, sexual violence, intimate partner violence, and stalking. Check out our Title IX for more information.
